(this means a client needs to be able to walk this distance safely). In order to deem a client eligible for a more supportive transport method a Medically Appropriate Mode Form (MAM) needs to be completed and SIGNED by a licensed professional (MD, RN, LCSW). MAM forms can take up to 15 days to be approved/denied by VEYO. If Denied, Veyo sends a letter in one business day to the client for a reason. If approved, MAM is logged in the client's file and they use the medically necessary mode of transportation going forward. These forms expire and need to be completed again after 1 year. If within the 15-day approval, time frame the client needs medical transportation, the client can ask for 1 to 15 day “courtesy” (up to 2 times in a row) to get this transportation mode while MAM is pending. If the client requires an escort/companion, a Companion Form must be completed by a licensed professional and approved by VEYO. For clients with appointments a distance away need the Closest Provider Form. This form is needed based on the following: 1) If they live in an urban area, the form is needed for appts beyond 10 miles 2) If they live in a rural area, the form is needed for appts beyond 20 miles. FOR QUALITY CONCERNS: Report Missed Pick Ups, Poor Customer Care, Long Hold Times, etc to which is their quality assurance department who will do an investigation. Include the date and time of the call as well as the agent- as the call is recorded and the investigator will pull this. Also asked to be included in the communication in the email as a representative for the client – 1) Include name of driver if known, or type of transmit 2) IDPs are Independent Driving Providers (like an uber) 3) 3POs are 3rd party operators- like ACE, Metro. Clients can ask in their record for a preferred provider if they prefer someone who has provided satisfactory services. If an appointment is scheduled/confirmed and the driver is a no-show, the main number can be called and select the option for “Where’s my Ride”. Report the driver is a no-show and you want to file a complaint so this can be logged in the driver’s file. If the ride is urgent, ask for a rescue trip to be pursued. Having a job provides us with feelings of self-worth, responsibility and independence. It nurtures our sense of accomplishment. Common Challenges . Trust us to provide the highes CT PCA PERSONAL CARE CONNECTICUT ALLIED CT PCA PERSONAL CARE CONNECTICUT CT PCA PERSONAL CARE CONNECTICUT CT PCA PERSONAL CARE CONNECTICUT ALLIED 1/6 APPLY HERE The Personal Care Attendant (PCA) program provides personal care assistance (attendant) for adults aged 18 to 64 with chronic, severe, and permanent disabilities. This program allows the adult to remain at home instead of institutionalization (placement in a long-term care facility, nursing home, etc.). A personal care attendant (PCA) is hired to assist you in performing activities of daily living (ADLs) and staying at home. There is a waiting list for acceptance into this program. Eligibility for PCA is a two-part process: Part 1: Functionally, you must need these services. Specifically, you must physically demonstrate that you need hands-on assistance in performing 3 of the seven core Activities of Daily Living (ADLs). These ADLs are outlined below: Bathing – need help bathing yourself properly? Dressing – need help getting dressed appropriately? Eating/Feeding – need help feeding yourself properly? (This does not mean making or preparing meals.) Toileting – need help going to/from the toilet and adequately cleaning yourself afterward? Transfer – need help safely transferring in and out of chairs/bed? Medication – need help preparing and taking your daily medicines? Behavioral – need daily supervision to keep from harming yourself or others? Part 2: Financially, you must have qualified for Medicaid by the time you receive services. You don’t have to be within the financial limits of Medicaid while on the waiting list for PCA (see below), but you must have applied and qualified for Medicaid by the time your name comes up on the waiting list. MFP ONLINE APPLICATION - CT MFP - CONNECTICUT MONEY FOLLOWS THE PERSON PROGRAM - MOVING FROM HOSPITAL TO HOME What is the MFP Program and what does it do? The MFP program helps a person with the funding and organization of moving out of a medical facility. It helps with setting up housing, necessities, medical equipment and temporary in home caregivers. MFP is a federal demonstration grant, received by the CT Department of Social Services from the Centers for Medicare and Medicaid Services. It was awarded to help rebalance the long-term care system so that individuals have the maximum independence and freedom of choice regarding where they live and receive care and services. The program builds on current programs by offering enhanced community services and support to those who have resided in nursing facilities for at least three months. Under MFP, CT will receive, for those transitioning back to the community, an enhanced Medicaid match of nine million dollars over five years, being reimbursed for 75 percent of costs for the first year back in the community instead of the customary 50 percent. This federal support is a financial incentive for Conn. to reduce the use of more expensive institutional care for Medicaid recipients. The approach is more cost-effective for taxpayers and is expected to lead to improved quality of life for older adults and people with physical and developmental disabilities and mental illness. People transitioning back into the community have the choice of where they want to live, whether it’s the person’s own home, a family member’s home, an apartment or congregate living. Goals of the MFP Increase dollars spent on home and community based services. Increase the percent of people receiving their long-term services in the community relative to those in institutions. Decrease the number of hospital discharges to nursing facilities for those requiring care after discharge. Increase the probability of people returning to the community within the first three months of admission to an institution Transition individuals out of institutions and back into the community Individualized care plans are created based on need.
