Colorado's Brain Injury Waiver can provide home and community supports for some eligible Health First Colorado members with traumatic or acquired brain injury. A local Case Management Agency helps assess the program route.

  3. If services were denied, reduced, delayed, suspended, or stopped: follow the notice and act before the deadline shown.

Official program and application route

The official Colorado Brain Injury Waiver brochure directs Health First Colorado members to contact their local Case Management Agency and directs people who are not members to contact the agency or apply through Colorado PEAK.

Ask the Case Management Agency for the current eligibility criteria, assessment, service options, decision, and accessible copies. A diagnosis alone does not guarantee waiver eligibility.

If you disagree with a decision

The Health First Colorado appeals page explains eligibility, service, health-plan, formal-hearing, expedited-hearing, and continuation-of-coverage routes. Deadlines differ by decision type. Follow the written notice, request the records relied on, and act immediately if existing services may stop.

What to save

  • The exact decision, service, assessment, or accommodation requested.

  • Every notice, service plan, assessment, email, fax receipt, and call reference number.

  • The deadline and effective date printed on the notice.

  • How the decision affects safety, health, communication, mobility, work, or community living.

  • The name and contact information of every agency, plan, case manager, and representative involved.

Communication support

Ask for plain language, extra processing time during calls, written follow-up, an authorized representative, an interpreter, large print, audio, Braille, or another effective communication method. Put the request in writing when possible and keep a copy.
