Use this directory when you know the problem but are not sure which official office is responsible.

Quick answer: Choose the office by issue, not by the result you hope to obtain. Read the agency’s own jurisdiction and submission rules before sharing information.

Pay, work time, and overtime

Connecticut Department of Labor: wage and workplace standards information and Connecticut complaint forms.

U.S. Department of Labor Wage and Hour Division: federal hours-worked, overtime, recordkeeping, and home-care guidance.

Workplace hazards

Occupational Safety and Health Administration: worker-safety guidance and the federal safety-and-health complaint route. OSHA’s home-healthcare resources address driving, violence, lifting, biological hazards, and unsafe conditions.

Licensed professionals and facilities

Connecticut Department of Public Health: practitioner complaint information and the Facility Licensing and Investigations Section for regulated health-care facilities. Confirm jurisdiction first. DPH states that submitted complaint information may be disclosable under state freedom-of-information law.

Help routing a health-care complaint

Connecticut Office of the Healthcare Advocate: public guidance on where different types of health-care complaints may belong.

Medicaid program integrity

Connecticut DSS: suspected client, provider, or vendor fraud or abuse involving programs it administers.

Connecticut Medicaid Fraud Control Unit: matters within the unit’s current Medicaid fraud and patient-abuse or neglect authority.

HHS Office of Inspector General: potential fraud, waste, abuse, or mismanagement in HHS programs.
