The Illinois Persons with Brain Injury waiver page explains who may qualify, available services, and how to contact the Division of Rehabilitation Services. The program can include supports such as personal assistance, home-delivered meals, emergency response, respite, nursing, therapies, environmental adaptations, equipment, day habilitation, pre-vocational services, supported employment, and cognitive behavioral therapies, subject to eligibility and authorization.

If you disagree with a decision
For many Medicaid benefit decisions, Illinois offers an online appeal route through the Manage My Illinois Medicaid page. Managed-care members may need to use the plan's internal appeal process before requesting a State Fair Hearing. Follow the notice that applies to your specific case.
Ask for the written decision and rule used, the assessment and records relied on, the exact appeal deadline, instructions for asking that current services continue if applicable, and an accessible format, interpreter, or disability accommodation.
Prepare the record
Keep the service plan, Determination of Need assessment, medical documentation, provider recommendations, correspondence, call details, and proof of submission. Write a short timeline showing what was requested, what happened, and how the decision affects safety and community living.
