Kentucky operates two Medicaid home and community-based waivers for adults with acquired brain injury: the ABI waiver and the ABI Long-Term Care waiver. This page connects the official application, service, and appeal starting points.

ABI and ABI Long-Term Care waivers
Kentucky's Acquired Brain Injury Branch explains that the ABI waiver supports adults who can benefit from intensive rehabilitation, while ABI Long-Term Care supports adults who have reached a rehabilitation plateau and need maintenance services to live safely in the community. Listed services include case management, counseling, environmental modifications, respite, supported employment, and residential supports.

The applicant or participant name and Medicaid ID.
The exact service, waiver, assessment, or accommodation requested.
The date requested and who received it.
Every notice, assessment, service plan, email, and call reference number.
How the decision affects safety, health, communication, mobility, work, or community living.
If you disagree with a decision
The Kentucky Medicaid Appeals and Complaints Branch manages member and provider appeals, administrative hearings, and state fair hearings involving decisions that deny or reduce Medicaid services. Follow the exact instructions and deadline on the written notice.
Ask for the complete written reason and rule used, all records relied on, an accessible copy, communication assistance, and written confirmation that an appeal or hearing request was received.
Communication support
You can ask the agency or health plan for plain language, extra processing time during calls, written follow-up, an authorized representative, an interpreter, large print, audio, Braille, or another effective communication method. Put the request in writing when possible and keep a copy.
