Brain Injury Services
Michigan's Brain Injury Services provides qualifying Medicaid beneficiaries with specialized post-acute rehabilitation and support. The official page explains eligibility and lists program contact information.
MI Choice home and community support
The MI Choice Waiver Program provides Medicaid-covered long-term services and supports in a home or residential setting for people who meet nursing-facility level-of-care criteria. MI Choice waiver agencies handle intake and service coordination.
If you disagree with a Medicaid decision
Michigan uses different hearing forms depending on the decision. The Medicaid Fair Hearings page explains the available forms. The MOAHR public-assistance page explains how to submit a hearing request for Medicaid enrollees, waiver applicants, and managed-care decisions.
Read the entire notice, identify who made the decision, check whether an internal plan or waiver-agency appeal must happen first, use the form named for that type of decision, attach the notice, keep proof of submission, and ask in writing about continuation of existing services if the notice says they will change.
Build a clear record
Save the notice, service plan, assessments, provider recommendations, prior authorizations, case-manager communications, and a one-page timeline. Describe the requested service and the functional or safety need in concrete terms.
Accessibility and representation
The MOAHR forms page includes a disability accommodation form. A person who needs help may also ask how to designate an authorized representative. Request plain language, written follow-up, extra time, or another effective communication method when needed.
