Pennsylvania Brain Injury, Medicaid HCBS, and Appeal Help

Pennsylvania has several government pathways for brain injury rehabilitation, Medicaid, and home and community-based services. This page gives you a short starting route and official sources.

Start here

  1. If you need brain injury rehabilitation: contact Pennsylvania's Head Injury Program or Brain Injury Help Line.

  2. If you need long-term help at home: review Pennsylvania Home and Community-Based Services (HCBS) and Community HealthChoices.

  3. If Medicaid services were denied, reduced, suspended, or stopped: read the notice, follow its appeal instructions, and act before the stated deadline.

Brain injury services

The Pennsylvania Head Injury Program pays for qualifying rehabilitation services and provides pre-enrollment help, assessment, service planning, rehabilitation, and case management. The official page lists the Brain Injury Help Line as 1-866-412-4755.

Medicaid and help at home

Pennsylvania calls Medicaid “Medical Assistance.” The Home and Community-Based Services page explains waiver-funded services and how to apply for long-term care and HCBS. The Community HealthChoices page explains the managed-care program for many adults who receive long-term services and supports.

Write down:

  • The applicant or participant name and Medicaid ID.

  • The exact service requested.

  • The date requested and who received it.

  • Every notice, assessment, service plan, email, and call reference number.

  • How the service affects safety, health, communication, mobility, or community living.

If you disagree with a decision

Use the instructions and deadline in the written notice. Pennsylvania's DHS hearing and appeal page explains that denials, reductions, suspensions, and terminations of Medicaid and level-of-care decisions may qualify for a fair hearing.

Ask for the complete written reason and rule used, the records relied on, an accessible copy of the notice, a language interpreter or disability accommodation if needed, and written confirmation that an appeal was received.

Communication support

You can ask the agency or plan for plain language, extra processing time during calls, written follow-up, an authorized representative, an interpreter, or another effective communication method. Put the request in writing when possible and keep a copy.
