Tennessee Brain Injury, CHOICES, TennCare, and Appeal Help
Tennessee's CHOICES program provides long-term services and supports for qualifying older adults and adults with physical disabilities. This page gives a short route for people living with brain injury who need help at home or need to challenge a TennCare decision.

If healthcare was denied, changed, stopped, or delayed: file a TennCare medical appeal within the deadline in your notice.
CHOICES and home support
The official TennCare CHOICES page explains that CHOICES can provide nursing-facility services or home and community-based services for eligible adults with physical disabilities and older adults. The page lists application help through Area Agencies on Aging and Disability and current health-plan contacts.
The Tennessee Department of Disability and Aging also describes home and community-based services and the OPTIONS program for eligible adults with physical or cognitive disabilities.
Write down:
The applicant or participant name and Medicaid ID.
The exact service, waiver, assessment, or accommodation requested.
The date requested and who received it.
Every notice, assessment, service plan, email, and call reference number.
How the decision affects safety, health, communication, mobility, work, or community living.
If you disagree with a TennCare decision
TennCare's medical appeal page says a member may appeal when requested healthcare is denied, changed, stopped, delayed too long, or not paid as expected. The page states a general 60-day appeal deadline and lists free appeal help at 1-800-878-3192. Follow the notice because a particular situation may have additional or shorter steps.

Communication support
You can ask the agency or health plan for plain language, extra processing time during calls, written follow-up, an authorized representative, an interpreter, large print, audio, Braille, or another effective communication method. Put the request in writing when possible and keep a copy.
