Texas Brain Injury, STAR+PLUS HCBS, Medicaid, and Fair-Hearing Help
Texas does not use one single brain-injury pathway for every person. Adults with disabilities may receive Medicaid health services and long-term services through STAR+PLUS or another program based on age, eligibility, assessment, and individual need.

Official program and application route
The official STAR+PLUS page explains that STAR+PLUS serves many adults with disabilities or adults age 65 or older. Its HCBS program can provide supports for qualified people who would otherwise need nursing-facility care.
Ask the health plan or service coordinator for the complete assessment, individual service plan, written eligibility decision, and the specific rule used. The official page lists 2-1-1 as an application route for Medicaid.
If you disagree with a decision
Follow the notice and contact the health plan or HHSC immediately. Texas HHSC's appeals guidance confirms the right to appeal Medicaid eligibility decisions. STAR+PLUS fair-hearing procedures describe hearings for waiver or service denials. Ask whether services can continue and request the evidence packet.

The exact decision, service, assessment, or accommodation requested.
Every notice, service plan, assessment, email, fax receipt, and call reference number.
The deadline and effective date printed on the notice.
How the decision affects safety, health, communication, mobility, work, or community living.
The name and contact information of every agency, plan, case manager, and representative involved.
Communication support
Ask for plain language, extra processing time during calls, written follow-up, an authorized representative, an interpreter, large print, audio, Braille, or another effective communication method. Put the request in writing when possible and keep a copy.
